Disclosure Log 2011 - 2012

received by West Midlands Ambulance Service. Log
details give the reference number, request details and the
response given to the applicant.
In some instances, details of a request and its response have
been omitted from this page to ensure patient confidentiality in
accordance with Caldicott Guidelines.
