All requests from the Police for, crew details, audio recordings of 999 calls and logs from 999 calls, Electronic Patient Records, Vehicle data, CCTV and Body Worn Camera data should enter the Ambulance Service by the one pre-arranged route.

The following documentation is required before the process starts:
- A Police Force recognised “Personal Data Request Form” with two wet signatures, one of which must be a Sergeant or Inspector. (the forms are available from the relevant forces Data Protection lead).
- If a second wet signature is unable to be obtained, the authorised officer can be CC’d into emails as authorisation.
- Signed Consent / Statement from patient, next of kin, parent or guardian for release of medical notes if requesting medical notes.
Without this information the Trust is unable to proceed with the request.
Requests for crew details are answered by Police Liaison and then the relevant Operational team can arrange accommodation for statement taking / Police interviewing sessions.
