All requests where the Solicitors are requesting their clients Medical Records, call recordings, statements and interviews with the crew are all required to complete the Trusts mandatory online form linked below;
Solicitor Subject Access Request Form
The following information is required on the forms for the process to begin;
- Organisation Name
- Lead Case Handler
- Reference Number
- Postal Address
- Contact Telephone Number
- Email Address
- Contact Preference
- Client/Patient Name
- Client/Patient Date of Birth
- Incident Date or Date Range
- Incident Location or What Three Words
- Confirmation of Litigation (Confirm if there will be action against the Trust or not)
- Signed Consent from client or appropriate legal person (e.g. litigation friend)
Once the form has been completed and submitted, you will be prompted by an acknowledgement email to submit relevant consent documents quoting the submission ID Number – these must be sent to [email protected]
Without completion of this form and submission of relevant documentation, the Trust will not take forward your request.
The following charges are applied to Solicitor Requests;
- Statements – £70 per individual statement (for 2 clinicians, this will be a cost of £140).
- Interview per Crew – £350 per hour (this is a standard charge for 1 or 2 clinicians)
